Online School Forms Now Available

Starting today, Tiger Families can access and update their online back-to-school forms.

Back-to-school forms give parents direct online access to update and change the following through our online parent portal OneView:

  • Update student contacts. Who should we contact in the event of an emergency? Parents can add or remove who is eligible to pick your student up during the school day. Please note that ONLY the friends and family listed on OneView are able to pick your student up during the school year.
  • Update emergency medical information. Let us know if your student’s physician has changed, if there are new medical orders from your child’s doctor such as the discovery of a new allergy, is taking new medication, or if your child wears glasses now.
  • Sign off annual disclosures. Each year, families are required to sign off on annual agreements such as the student handbook, photo release (to be highlighted and have their picture taken for the newspaper, social media, website, etc), interact with the school therapy dog, and more.
  • Update your address. Have you moved over the summer? Let us know for transportation and safety purposes.

If you have no previously logged in to update your student’s OneView back-to-school forms, you must first create a parent account and then link your student to your account to update their forms. This can be done by calling the district office over the summer 740-474-4340 to verify your information over the phone to receive your student’s ID number to link accounts.

In instances of shared parenting, ONLY the residential guardian/parent is able to modify student emergency medical information. Secondary accounts can be created to view all information added to the back-to-school forms upon request to the school, but these accounts will only have ‘view only’ permissions.

ALTERNATE TRANSPORTATION FORMS (DUE JULY 9)

New this year. Alternate transportation forms are now hosted online in a Google Form and were due on Friday, July 9th. Alternate transportation forms allow families to choose one additional address to transport their child to daily. Once again, this year there will be NO day variant busing meaning families cannot have different addresses to bus their child to on different days of the week. Only one address other than the child’s residence will be permitted within district boundaries.

Athletics Registration in Final Forms

Student-athletes in grades 7-12 who plan to participate in sports this year are required to complete all waivers and medical information prior to the start of their season. The only form that you are still required to actually print, complete, and file in the Athletic Department, is the actual OHSAA Physical Form (download and print here). The physical form must be completed by the doctor and by law we are required to have a signed copy on file.  Once your student’s physical is received by the athletic department we will input the expiration date into Final Forms and then you will receive automatic notifications when your student’s forms need to be updated.
