Second vaccine clinic to be held June 15th
On Tuesday, June 15th Circleville City will hold its second Pfizer Vaccine Clinic at the high school from 10-11 a.m.

If your student participated in the first age 12-18 Pfizer vaccine clinic last month, our second clinic hosted at Circleville High School will be held from 10 a.m. to 12 p.m. on Tuesday, June 15th. Students are asked to arrive promptly between 10-10:30 a.m.

About the clinic, a few items to note:

If your student did NOT participate in the May clinic to receive their first dose and would like to receive their 1st dose at this clinic they MAY do so by emailing our school nurse at [email protected]

Please note: The June 15th clinic will be the last on site clinic hosted at Circleville City Schools this summer. If you elect to sign your Tiger up for their first dose of the vaccine you will need to pursue one of the frequent clinics offered by the health department for their second dose three weeks from this date.

If the June 15th clinic does not work for your family’s schedule to get your child here from 10 a.m. to 11 a.m. the Pickaway County Health Department offers OTHER clinics available on different dates and times. In the event transportation is an issue in getting your child to school, please email the school nurse to request transportation at [email protected].

Finally, as was the case at our May clinic, there are three forms to complete and return with your student on June 15th. These can be found below. These forms/waivers must be completed for your child to be eligible. In instances where printing is an issue, our front offices will have these forms available next week upon request.
